To receive your Rebate Check by mail,
please follow these simple steps:

  1. Purchase a set of four qualifying tires from the list below between November 1 and November 29, 2008.
  2. Enter your ZIP code.
  3. Please enter your 16-digit Goodyear Credit Card account number. If you have recently applied for the Goodyear Credit Card and are approved, you will receive your Goodyear Credit Card number by mail within 7 — 10 days of approval.

    By responding to this offer you will be disclosing all the information requested above, including your credit card account number, to a company outside the Citigroup family of companies.

  4. Send a copy of your invoice and this Mail-In Rebate form to:
